Consultation Request Form
What Each Step Means
1. Initial Inquiry
Families or individuals reach out to us via phone, email, or website to ask about our services. We respond with friendly guidance and help determine whether our support fits their current needs.
2. Booking Appointment
Once interest is confirmed, we schedule an initial session (in person or online) to learn more about the participant’s strengths, needs, and goals.
3. Initial Consultation
In this first meeting, we listen carefully to the family’s concerns, observe the child’s current abilities, and gather information to help us design a non-clinical support approach. Note: This is not a diagnostic or clinical assessment.
4. Support Plan
Based on the consultation, we provide a simple, strengths-based support strategy. This plan may include suggestions for daily routines, visual aids, activity ideas, or family guidance. This is not a behaviour support plan under NDIS rules.
5. Ongoing Assistance and Follow-up
Our team offers regular check-ins or continued support to adjust strategies, provide encouragement, or assist with new goals. Frequency depends on the family’s needs and preferences.
6. Feedback & Review
We welcome your feedback and offer review sessions to reflect on what’s working, what can be improved, and how we can best continue to walk alongside your family.
